ダイビングプログラムに参加するにあたり医師の評価を受ける必要があるかどうかを判断するチェックシート。該当時は3枚目の医師による評価シートをご準備下さい。

・成年用

・中学生以上未成年者用

​・小学生以下用

This is a confirmation form for snorkeling participants.

・ Adults

・ junior high school students       and up to the age of 18 only

・ children under junior high            school level
